Your visit notes
Take a short question sheet to the visit
Write your goal at the top of the sheet. Bring the offer you are considering and leave room to write down the answers from the person treating you.
Put the offer in context
Make a note of medicines and supplements you already use, questions about your health, and what you hope the visit will resolve. Bring any records you want to discuss.

The sheet also works with your browser’s Print menu.

Answers and next steps:
Close the visit with an answer you can repeat
Try putting the plan back into your own words. Ask for a written copy of anything that still feels unclear.
For the money question, ask for a total that names the visit, medicine, tests and any recurring fees. Keep it with your question sheet.
